Registration
STEP 1 – Documentation
Please gather the following required enrollment documents and email them to admissions@lilliemay.org
a. Student’s Birth Certificate/Passport
b. Updated Immunization Records (Rising 7th Graders need updated shots!)
c. Parent/Guardian Photo ID
d. Two Proofs of Address
e. Completed Enrollment Form (below)
f. A copy of the most recent physical (within 6 months) or a completed Health Assessment (attached below).
STEP 2 – Registration
Option 1 – Complete the Student Enrollment Form (attached below) and email it along with enrollment documentation to admissions@lilliemay.org
Option 2 – Register in-person: Head to Lillie May Carroll Jackson Charter School at 2200 Sinclair Lane, Baltimore, MD 21213 during summer business hours 9 AM to 3 PM, Monday – Thursday.
